Need a Family Doctor?

Dr. Reza Yaghini, M.D., C.C.F.P., is now accepting new patients in The Blue Mountains Community Health Centre at 78 King St. East, Thornbury.

If you have a current family physician but wish to change, you have to call Health Care Connect at

1-866-532-3161. or 1-888-218-9929

You will be asked to de-register with your current family physician and then to call

1-800-445-1822 so you may be referred to Dr. Yaghini. You will be asked a short series of questions.

If you do not have a current family physician, you have to call Health Connect at 1-800-445-1822 in order to be referred to Dr. Yaghini.
